Program Name: Family Case Coordination Program–
Description:
An intensive home visitation and case management program serving families affected by substance abuse who qualify under the Child Abuse and Prevention Treatment Act (CAPTA) in Philadelphia.  HFP is one of two agencies providing services to CAPTA families in Philadelphia.
Referrals: Families are referred by the Philadelphia Department of Human Services, Division of Community-Based Prevention, after an assessment is conducted by the CAPTA Unit. 

Goals:

  • To reduce the incidence of relapse among mothers post-delivery through provision of health and social service case management support services; and 
  • To achieve family stability through enhancement of parenting skills and mother’s awareness of the well-being and needs of her children.

Services are provided on a voluntary basis for three months.
