The goal of these programs is to reduce the health risks of mothers, infants/children and families experiencing barriers to care and psychosocial and environmental stress. Each participating health center employs a team of nurse or social worker supervisor and lay home visitor(s) to provide services to its patients. The Division of Early Childhood, Youth and Women's Health, Philadelphia Department of Public Health, funds these programs.
