Pediatric Enhanced Detection

This project identifies children ages 0 – 6 at potentially disproportionate risk for child abuse/neglect, developmental delays, failure to thrive, elevated blood lead levels, delayed immunizations and other factors by using missed well-child appointments as an indicator. A multidisciplinary team including a clinical team leader,  pediatric case manager and program assistant facilitate the internal case finding, team meetings and interventions (letters, phone calls, home visits, and case management services, including referrals to other providers) to children and families enrolled in the program. The program is based on a program at Good Samaritan Hospital in Lebanon, PA. In Philadelphia, the program has been implemented at two District Health Centers.

Goals:

  • Early identification of children at increased risk

  • Early intervention with identified children, including increased engagement with parent/caregiver

  • Risk reduction and increased adherence to preventive pediatric protocols

Program Eligibility:

Children ages 0 – 6 who are receiving pediatric care at Health Centers # 5 and # 6 with: 

  • A pattern of missed well-child visits, and 

  • One or more of the following potential risk factors for poor pediatric outcome:

    1. deficient well child and/or immunization history

    2. weight and height issues

    3. medical neglect (lack of comprehensive follow up medical care)

    4. child abuse/neglect

    5. parental domestic violence

    6. current parental drug and/or alcohol abuse

    7. current parental incarceration

    8. parenting issues

 Capacity:

A total of 75 children at Health Centers #5 and #6 will be enrolled in the program at any given time.
