Community Health Centers, also known as Federally Qualified Health Centers or FQHCs, offer high quality comprehensive primary care at low-cost in many communities in our area.
FQHCs offer a full range of primary care services for adults, children and seniors. Additional services often include prenatal care, dental care, mental health services, prescription drugs, and insurance enrollment.
HFP is working with health centers and other organizations in Southeast PA to help people enroll in the Health Insurance Marketplace, now available as part of the Affordable Care Act/Obamacare. The HFP Navigator team are placed in the City’s District Health Centers, where both current patients and non-patients can work with them to enroll in the Marketplace insurance options. In addition, the Navigators will work with a range of community and faith-based organizations to provide information to consumers and help them to enroll.
For more information, to ask about having a Navigator at a community event, or to seek direct assistance from an HFP Navigator, please call 215-977-7255 or email email@example.com.
In addition to resources available through HFP, all community health centers have staff who are trained to enroll people in Marketplace insurance options. To locate a community health center near you, please visit our HFP Member Health Center Locations map
