Home Visitation (215) 977-8999 hhughes@healthfederation.org The goal of HFP's Home Visitation Program, which was begun in 1988, is to promote access to high-quality health care and other needed services for low-income pregnant women, children and families. These families face a number of challenges including economic and cultural barriers to care, social isolation, lack of information, HIV/AIDS, homelessness, chronic illness, substance abuse and violence. HFP home visiting services are provided directly through its division of Children, Youth and Family Services and through sub-contracts with its member community health centers. Home visitors are recruited from program communities and are trained by HFP to offer family support, practical information and referrals to other resources. Nurses and licensed social workers supervise the Home Visitors and help coordinate home visiting with health care and other services, provide clinical interventions, and ensure consistent and continuous care. All program staff members participate in ongoing professional development activities and receive technical assistance through HFP. The Home Visitation Program has been successful in improving the health and well-being of families and communities.
